MENGISI FORMULIR PENDAFTARAN ONLINE
1) Sebelum memulai pendaftaran, perhatikan beberapa hal berikut, Siapkan dokumen berikut ini dalam bentuk softcopy untuk diunggah (upload) :

• Dokumen Rapor ( Hanya bagian yang ada nilainya )
  kelas 10 semester 1 & 2
  kelas 11 semester 1& 2
  kelas 12 semester 1 (optional)
  *Dokumen Rapor harus dilegalisir (cap basah) resmi oleh sekolah.
  *Ukuran file maksimum 1 MB
• Fotokopi KTP/kartu pelajar/paspor
  *Ukuran file maksimum 1 MB
• Pasfoto 3×4 berwarna (warna latar belaksang bebas)
  *Ukuran file maksimum 1 MB

2) Bagi peserta yang ingin melakukan tes ulang, klik Test-Ulang, isi formulir, kemudian silahkan unggah (upload) bukti pembayaran sebelum batas akhir pendaftaran ke Bagian Admission Universitas Prasetiya Mulya Kampus BSD
